Answer:

It is important to not touch your face due to germs particles potentially getting into your mouth, nose, or eyes. For example, lets say you were at the supermarket and you touched a cart that someone who was sick left their germs on. Those germs cannot enter your system unless greeted with an open orifice. Summative evaluation requires both the measurement of a health indicator 1) at baseline and 2) after the program has concluded.
mezya [45]

True :- evaluation requires both the measurement of a health indicator.

What is health indicator?

Health indicators are quantifiable traits of a population that academics use to substantiate claims about the population's health. Typically, researchers will utilize a survey methodology to learn more about specific individuals, then use statistics to try to generalize the findings to the entire population. Finally, they will use the statistical analysis to draw conclusions about the population's health. Health indicators are metrics created to condense data on important issues pertaining to population health or the effectiveness of the healthcare system. They offer comparable and useful data that can be applied across various administrative, organizational, and geographic borders and/or can monitor development over time. Attention, warning, and alarm are the three different categories of alerts.

If a health indicator detects a non-normal state for one of the different states it measures, an attention alert is sent out.

the nurse obtains a client's history that includes right mastectomy and radiation therapy for cancer of the breast 10 years ago.
AnnZ [28]

Pathologic fracture of two ribs on the right chest should be the current health problem that the nurse should consider as a consequence of the radiation therapy.

The appearance of two right-sided ribs with pathological fractures without evidence of trauma is associated with radiation injury because ribs lie in the radiation pathway and become less dense, thinner and more fragile over time.

Radiation therapy (also called radiation therapy) is a cancer treatment that uses high doses of radiation to kill cancer cells and shrink tumours.

Low dose X-rays use radiation to look inside the body, such as his X-rays of teeth and fractures.

The most common early side effects are fatigue and skin changes. Other early side effects are usually related to the treated area and include hair loss and mouth problems when that area receives radiation therapy.

Delayed side effects may take months or years to develop. They can appear in normal tissues in the body that receive radiation. The risk of side effects after depends on the site treated and the radiation dose used.

A careful treatment plan can help prevent serious long-term side effects. It is always recommended to consult a radiation oncologist about the risk of long-term side effects.
