D is a prime polynomial. 

The best way to figure this out is by attempting to factor them all. Each of them factors into perfect squares except for D. 

A)
x^2 - 6x + 9 = (x - 3)^2

B) 
x^2 + 16x + 64 = (x + 8)^2

C)
4x^2 + 20x + 25 = (2x + 5)^2

D we can also tell won't have a perfect square like that because it ends in a negative number. No number multiplied by itself is negative.

The equation of a line starting from two points is:
y-y_1=\frac{y_2-y_1}{x_2-x_1} \cdot (x-x_1)

From the first point you get: x1 = -1, y1 = -2
From the second point you get: x2 = 3, y2 = 10

Replace x1, y1, x2, y2 in the equation of the line and you get:
y+2=\frac{10+2}{3+1} \cdot (x+1)
y+2=\frac{12}{4} \cdot (x+1)
y+2=3 \cdot (x+1)

From this you get the equation of your line:
y(x)=3x+1
