Why is it recommended that a patient’s name, DOB, and chart number/medical record number appear on each page of a multipage form

<span>This ensures that the the information on the page is being attributed to the correct patient. If a page were dropped or accidentally mixed into another patients forms, it would be easily noticeable who each page of the form belongs to.</span>
