Usually "Peptic", this is not so much a bleeding ulcer (which is usually a Gastric Ulcer). but is painful.

Your answer will for sure be, B. Provide inadequate training to workers about power line hazards because the word inadequate means not enough, bad, not useful, and more negative things... Everything in B has a good connotation except for that one word. They'll end up dead with that type of training.

Why not the rest?
A- it states to "locate" before digging which is a good precaution.
C- the word "prevent" infers to stop something bad from happening and the rider posts is another good precaution.
D- the word "verify" is another word inferring to prevent something bad form occurring. The word "visibly" stands for assurance. Assurance that the owner has de-energized and grounded the power lines. Who wants to get electric shocked?
All except for B has a good connotation meaning all except B has something or stands for something good.
