Urine forms as some of the blood plasma spills out, some of these spilled plasma components are taken back up into the blood, and other material is selectively sent out to be added to the urine. In other words, there are three main things happening to form urine. 
glomerular filtration (blood plasma spills out) bullet tubular reabsorption (some items are selectively reabsorbed into the blood) bullet tubular secretion (some blood plasma items are selectively sent out into the tubules)

Mutation, is generally, any change or alteration to the genetic sequence of a DNA. However, mutation can be of different types depending on how it occurs. According to this question, a mutation is said to be SILENT if it involves the substitution of a nucleotide with another nucleotide that does not change the protein molecule encoded by the gene. In other words, the mutation happens in such a way that both the original sequence of gene and the mutated one codes for the same amino acid sequence (protein).

On the other hand, a mutation is NON SILENT i.e missense or nonsense mutations, if the mutation causes a change in what amino acid the DNA strand codes for. For example, in missense mutation, a different amino acid is produced while in nonsense mutation, a STOP codon is produced.
