The point-slope form of the equation through those points is
.. y -1 = (1/4)(x -12)
Multiply by 4 and subtract the left side to put into general form.
.. x -12 -4(y -1) = 0
.. x -4y -8 = 0
Add 8 to put into standard form
.. x -4y = 8
