While glucose is filtered too, it will be removed by the kidney cells up to a certain point. In diabetes, the glucose level is too high that there is some glucose left in the urine, lead to glucosuria. 
Glucose will attract the water, makes the urine is hard to concentrate and have a higher volume than normal, leading to polyuria.
