R = $11.50/hr
H = 40 hrs per week
D = 12 weeks
Extra = $500

Find how much he earns per week
$ 11.50 x 40 = $ 460.00

How much did he earn in 12 weeks
$ 460.00 x 12 = $ 5,520.00

Add his wage and the extra
$ 5,520.00 + $ 500 = $ 6,020.00
