The y-intercept is the part were the line touches the y axis, which in this case you can see is the five

To find the slope you need to use two points. I used (0,5) and (4,6).

This part is kind of tricky to explain so I kinda hope you already know this and sorry if it doesnt make sense but the slope needs to be written as rise/run which could also be reffered to as difference in y/difference in x. So basically what I did is subtract the difference in y values, which is 1 and the difference in x values, which is 4. And then I just put the difference in y value over the difference in x value
