The  action that is example of someone taking action to accept personal responsibility for eating right is: Writing down foods consumed at each meal in a day.

<h3>What is personal responsibility? </h3>

Personal responsibility for eating right is the right that enables us to make right choice when it comes to food we consume  so as to choose food that are healthy or food that will nourish our body.  

Writing down the foods we consumed on a daily basis will help us to known how healthy or unhealthy the foods we consume are.

Consuming good meals that are well balanced per day and making use of  healthy eating time table will help to improve our well being which inturn will help to reduce the risks of developing some disease.
