There are no choices here but the reason why it is the duty
of the state government to protect public health is because it starts at the
grass-roots level where municipalities as well as city and state officials are
the ones responsible for directly monitoring and implementing preventive
measures to stop the spread of diseases.

They were the lower class in Medieval times. They barely had crops for themselves and/or made very little money, even though they worked really hard. They were capable of leaving, but they stayed in their home land. Also, due to all the hard work, most serfs wasn't expected to live long.
