When elevated for swallowing and sucking, it completely blocks and separates the nasal cavity and nasal portion of the pharynx from the mouth and the oral part of the pharynx.

the appropriate tone to use in this instance would be a stern but kind and understanding tone. you don't want to be too forecul in order to avoid the patient rebelling however, you must be stern since it is a matter of life and death; the best course of action is to be entirely honest about the serious effects his habits have on his health. Based on his past actions of following the regimens prescribed by other doctors, he seems to want to lose weight and do better.
