IRB stands for Institutional Review Board which is establish to protect the welfare of people who are participating in human researches.

In this case, a research is conducted focusing on hygiene and prevention, but two weeks later a subject had a heart attack. So, in this scenario, the researcher doesn't have to report this event to the IRB, because the research is not related with this type of disease, it's only about prevention, and there's no way that any event could help to have a heart attack.

Therefore, the report is not needed, because what happened is due to other variables outside the research.

This should not be reported to the IRB or the Institutional Review Board. Since it is not related to the research about hygiene and disease prevention that was conducted two weeks ago. And also, even though heart attack is a disease, it only happens if there is inadequate supply of oxygen-rich blood to a part of the heart muscle, then it suddenly becomes blocked and the heart can't get oxygen.
