so
we see the verttex is at (4,-1)
so
h=4 and k=-1
y=a(x-4)²-1

find a

use a point
(3,0)
x=3 and y=0
0=a(3-4)²-1
0=a(-1)²-1
0=a-1
1=a

so da equation is
y=1(x-4)²-1
expanded we get
y=x²-8x+16-1 or
y=x²-8x+15
