The small structures attached to the Rough ER, and thus is the reason as for why it is called "rough" are the presence of A. Ribosomes. These organelles responsible for protein synthesis are attached to the rough endoplasmic Reticulum, this also differentiates between the Smooth ER, for the inverse reason, they do not have ribosomes attached.
