The first factor is "physical arousal" or physiological response as a result of an environmental stimulus. The second factor is then the cognitive response that tries to interpret the initial physiological response.

In the case of Sally, the first factor was her increased sweat and heart rate as a result of the exam, while the second was her cognitive processes which interpreted her increased heart rate due to the fact that she is unprepared for the exam.
