   x^{2} -4x+16 =0

solutions of this quadratic equation can be obtained by

   x=-b +- \sqrt{b^{2}-4ac } /2a

let use y for factors

x=-(-4x)+-\sqrt{(-4x^{2} )-4*x^{2} *16}  / 2*x^{2}

x=4x+-\sqrt{16x^{2} -64x^{2} } /2x^{2}

x=4+-\sqrt{16-64}/2
