It is false that all symptoms documented in the HPI are also documented in the ROS.

HPI stands for "History of Patient Illness" and is a description of the event of the patient's present health problem. The HPI is mostly a written account description of the progression of the patient's present health problem from the primary sign and symptom to the present.

The Review of Systems (ROS) is a listing of the body systems that's obtained through a series of queries so as to spot signs and/or symptoms that the patient could also be experiencing.

HPI focuses is story regarding the chief grievance and its associated symptoms whereas ROS is a listing of symptoms which includes chief complaints and symptoms.
