Plug in:

Slope = m = 4

Point = (x1,y1) ----> (-3,7)

y - 7 = 4(x - -3) //Solve for y

y - 7 = 4(x + 3) //Answer

y - 7 = 4x + 12

y = 4x + 12 + 7

y = 4x + 19
