I would say feelings of worthlessness. You can't really see it on their body, but they can tell you if they choose to, so that makes it hidden in my opinion.

Few jumping jacks before bed and when they wake up. Planks when they have free time. Go on the tredmill for 30 minutes a day. Try and lift weights for about 10 minutes a day.
