If I was the nurse I would say

“ your primary healthcare provider has most likely asked you to stop drinking because while it might not have caused a problem yet the effect of drinking 4 glasses of whiskey a day for 20+ years add up over time and could cause serious health issues in the future”

They are precontemplation, contemplation, preparation, action and maintenance. The preparation stage would be where you have decided to make a change and are preparing yourself to make that change, then you would act upon your decision. You would still need to maintain that change.
