Point slope formula has fallowing shape:
y-y1=m(x-x1)

where x1 and y1 are coordinates of the given point.

in f(x) = -5x + 2 we can see that slope is -5
m = -5

if we exchange all of this in point slope formula we get:

y - 12 = -5(x - (-2)) = -5(x+2
