Slope - 3 , coordinate point (3, -4)

Equation in point slope form:
y - y1 = m(x - x1)

So
y + 4 = -3(x - 3)

(y-5)=(-3)(x-0)

y-5=-3x

y+3x=5 is the required equation.
but, I dont know why there is so much information in the pic. more than what is required.
