Depending on the illness, the treatment could involve skillful treatment, restorative care, or rehabilitation. Transitional care is the assistance provided when a patient or client moves from one setting of care (such as a hospital, nursing home, assisted living community, skilled nursing facility, primary care physician, or specialist) to another.

Transitional care frees up beds for more seriously ill patients while also enabling patients to rehabilitate at home. Every day, three patients are on average referred to the program from Khoo Teck Puat Hospital.
