If Jake has never experienced these types of things before, as in something to do with body image, procrastination, motivation, etc. then I would think this is the correct one because of his age.
He may have a medical condition but everyone struggles with at least one of these things throughout their lifetime. so does that mean everyone who has ever thought about things relating to what Jake is going through has a medical condition or illness, etc.? No, no it does not.
They said in the paragraph he is prone, they didn't say that he was currently suffering from it. Therefore, since he can't tell how Jake is feeling internally this would probably be a big leap.
