There is no statements I can comment on, the only logical explanation is that her medication must've been way to expensive and she thought that heroin was a cheaper and more effective option rather than spending $180 for 30 pills at her local CVS she could get 180mg of stamps or drops for $30.

I would have consulted with the patient about if it was okay to let the other person know that they could now be infected with HIV and if the HIV positive patient said it was fine I would go back and tell the other patient.
