It may include information received by a healthcare provider, or others related; any information about physical health or mental health of a patient, whether from the past, present, or future; also any information related to any payment for health care. PHI may be considered in any form such as verbal, computerised or written.
The permissions for the use of this information are "for treatment, payment, and healthcare" procedures.

Diet books and supplements cannot guarantee long-term success. For starters everyone is different and what might work for one person might not work for another. Diet books tend to be a mixture of personal opinion and healthcare facts and it's difficult to know what to completely trust. Most of the books and supplements tend to be based on current trends also and many have turned out to be inefficient.

If you're very young or very old you could be misjudged for not being as capable as somebody in their 20s for example. While this may often be true it is not always and isn't something we should strive towards. 
If you're of a certain ethnicity; let's say you're a Kenyan and you're running a marathon, people could misjudge that you would run the marathon at a very good pace. This, again, is not the necessary case. 
If you're female, it is often possible that you would be misjudged as not being strong enough compared to your male counterparts.
