An older client recently has been taking cimetidine, the nurse monitors the client for <u>confusion</u> which is the most frequent central nervous system side effect of this medication.

A cimetidine is a histamine-receptor antagonist.

The main clinical manifestations are mental confusion, coma, etc.  It may be neurotoxic in debilitated patients especially when the blood-brain barrier is compromised.

It is so because cimetidine elapse the blood-brain barrier, and this increases the probability that mental status changes are due to blockade of central nervous system(CNS) histamine(H2) receptors. Cimetidine associated neurotoxicity may be characterized by signals of affective dysfunction, toxic delusional state, confusion.
