Answer: Squinting more while reading and difficulty reading street signs at night.

Explanation:

Chase has frequent sinus and because of that there can be difficulty in sleeping and frequent sinus headaches. As this problem persists he would not be able to read much and this could be a reason for failing grades in class.

But when he reads he squints more while reading and also has difficulty seeing objects at night. These symptoms shows that he is having some eye problem and needs medical assistance.

These symptoms refers to visionary problems and needs to get his eyes checked by the ophthalmologist.

Difficulty sleeping would most likely relate back to the frequent sinus headaches.

The only ones I see that are directly related to vision are
squinting more while reading

His failing grades in school would probably be due to all of the above.
