Contact the Occupational Safety and Health Administration (OSHA).  Rules vary according to the type of industry you are working in but they will advise you as to the proper forms to complete.  All industries with 250 or more employees are under OSHA automatically but small work places are also covered.  Its important to protect yourself from retaliation from your employer but don't be afraid to report the conditions.  The OSHA office will advise you of your rights and will protect you from retaliation and your employer will suffer consequences if he tries to cimcumvent the regulations imposed by 
OSHA.  The fines can be pretty stiff for the industry and you have legal rights as a whistle blower.  Go straight to OSHA...not your employer, they will handle everyting for you.
