The inhalation of smoke by a person other than the actual active smoker is called passive smoking. This type of smoke is called environmental tobacco smoke (ETS). It is also known as secondhand smoke (SHS). It occurs as a result of the release of tobacco smoke into the environment and its subsequent inhalation by the people present in the surrounding environment. Such environmental tobacco smoke is harmful to both adults and children. It can only be prevented by eliminating smoking in workplaces, public and at home.

The burning of tobacco-containing products like cigars, pipes, or cigarettes causes ETS which is inhaled by the smokers as well as the nonsmokers in the close vicinity. The smoke from tobacco contains about 7,000 chemicals which include toxic products that can even cause cancer. Nicotine and tar are the solid particles which contribute to about 10% of the tobacco smoke. The remaining 90% smoke is composed of vapors or gases.

Environmental tobacco smoke causes the following in children:

  1. Severe asthma attack
  2. Respiratory symptoms such as sneezing and coughing
  3. High risk of sudden infant death syndrome (SIDS)

Effect of environmental tobacco smoke in nonsmoker adults:

  • Heart disease
  • Lung cancer
  • Stroke

The death toll for nonsmokers since 1964 has reached 25, 00,000 due to exposure to ETS causing various health problems. This results in approximate 45,000 death of nonsmokers per year till now.

The amount of ETS in a person is measured by evaluating the cotinine content in the saliva, blood, or urine. The nicotine present in the tobacco smoke breaks down to form cotinine inside the body.
