S – Hi doctor, I am Ben, RN on AMU this side. I am currently caring for your patient Mr. O. I am concerned  because his vitals have changed and he has sudden abdominal pain with distention.  

B – Patient was admitted for syncope. Has a history of CHF and acute MI. Only medications patient  reports to take is the occasional NSAID for chronic back pain.  

A – The PCA reported to me that Mr. O is having black, tarry stools. He is having abdominal pain that he  rates as a 6 out of 10 and there is distention present. BP 94/66, pulse 114, and RR of 24, O2 is 97%. He   says he cannot get comfortable and is feeling unwell.  

R – I recommend that you come see the patient immediately, and that we remove any orders for NSAID   use and instead switch to a non NSAID such as acetaminophen. Fluids should be started to counteract  hypotensive state. Do you want any tests or labs done such as an abdominal CT scan or type and screen?
