Equation for linear function.

y=mx+b

We need 2 sets of numbers from the graph to find the slope. Do rise/run or y/x.

(50-28)/(5-4)=22/1=22

so m=22

y=22x+b

Now substitute x and y in to solve for b

28=4*22+b

-60=b

Now we have the equation all ready.

y=22x-60 Done!
