A 46-year-old, 230lb woman with a family history of breast cancer. She is up to date on yearly mammograms. She has a history of

HTN. She complains of hot flushing, night sweats, and genitourinary symptoms. She had felt well until 1 month ago and she presented to her gynecologist for her annual gyn examination and to discuss her symptoms. She has a history of ASCUS about 5 years ago on her pap, other than that, Pap smears have been normal. Home medications are Norvasc 10mg qd and HCTZ 25mg qd. Her BP today is 150/90. She has regular monthly menstrual cycles. Her LMP was 1 month ago.

Answer:

The patient needs lifestyle modification and should come for routine check ups and follow ups.

Explanation:

As given in the context, the patient has high blood pressure or is hypertensive. She also had a typical squamous cells of some undetermined significance. She also had a history of breast cancer in the past.

So the patient need medicines for hypertension and also needs modification in her life styles. She should regularly visit the doctor for health check ups.

I would recommend :

-- regular monitor the B.P of the patient and inform her B.P. level to her.

--   teach her some self health monitoring skills

--  modify the dosage and will change the medications since the patient is suffering from hot flushes and also genitourinary symptoms. These are the effects of medicine amlodipine as well as hydrochlorothiazide that is taken for reducing her BP.

For ASCUS, I recommend :

-- pap smear to be done yearly

-- cytology to be repeatedly done

Yearly mammography is to be recommended for breast cancer history.

Also advice to exercise for thirty minutes daily and reduce the salt content to less than 2.5 g in a day.
