I would say that the shifting of the material in the ambulance with a change in direction of the ambulance could be due to centrifugal force is turning a corner or also it could be conservation of momentum so that the force it had from the forward movement of the ambulance makes the material keep moving forward even though the ambulance has changed direction of movement,
