Failing a quiz, would cause distress and worry, primarily because of the fact that you have not prepared well and as a consequence worried how it will affect your grades and ultimately your career later on. Depending on how important that quiz is, in a very important course etc.

The correct answer would be failing a quiz.

Distress is bad stress as opposed to eustress which is good stress. Distress can be caused by anything that has a negative effect on the mind and body. Failing a quiz would cause you to feel negative and upset and possibly disappointed in yourself all of which are bad and negative.
