Your answer is choice D:

By equation, the form of a proportional relationship is y = kx
By graph, a proportional relationship can be noticed by observing the graph of the line passing through the origin.
By Table, a proportional relationship will satisfy the following: k =  \frac{y}{x} where k is the constant of proportionality.

In choice D, divide each y coordinate in the table by its corresponding x coordinate and you will see that the answer is 1.5 for each.. This is your k, or constant of proportionality.
