6) name/address of the food manufacturer, packer or distributor

Explanation:

The percent daily value, the serving size and calories of the food shall be included in the label. For example, daily calories needed by a healthy person are 2000 calories and 25% of vitamin in food label refer to 25% of daily calorie in the form of vitamin. Total fats i.e. saturated and unsaturated fats can also be included in the label.
