It dangerous to administer an intravenous line of pure water to a patient because the pure water will go directly into a vein would cause your blood cells to become hypotonic, possibly leading to death.

Serious patient harm can result once sterile water is run by direct IV infusion due to hemolysis related to the hypotonic nature of the product. Sterile water for injection is sterile water that is supposed to be used by pharmacy to compound IV product like canal nutrition solutions.

Pure water is injected into an IV would diffuse via osmosis into the more concentrated environment within red blood cells, create them intumesce as a result of the surplus water and eventually create them burst.
