You have a service user coming into the home. You have been asked by your manager to complete the assessment for this gentleman.

You have been informed that he has a substance misuse for alcohol. You have been asked to create a care plan to support this person with his substance miss use and monitor his intake. The alcohol nurse has advised that he must only have 4 cans of beer a day and this need to be monitored.
