The nurse might first notice that the patient's skin is not as elastic as it should be. This can be done through a pinch test. Pinching the skin and watching to see how quickly the skin regains its original surface structure. If the skins stays pinched and takes a few seconds to regain original surface structure, the patient is likely to be dehydrated.
I think the person got some "fats" in there serving and havent burn it through exercising so its stored in the body as body fat which will make the person gain weight.
