The desire for chocolate and belief that there is some in the fridge is a mental instance. The mental instance is limited upto brain only. The thoughts, perception, desire and feelings lies in the state of mind.
This belief has been now transformed into physical causation and because of this the door of fridge is opened to look for chocolates inside it.
This act is an instance of mental to physical causation.
