Solution:

The vertex form of a parabola is

y=(x-h)^2+k        ...(i)

Where, (h,k) is vertex of the parabola.

We have,

y=(x-4)^2+1         ...(ii)

From (i) and (ii), we get

h=4, k=1

The vertex of the parabola is (4,1).

Now, the table of values is
