Question

A new patient visits the dental office, and the business assistant asks the patient to fill out a registration form. Then the dentist examines the patient and explains a plan of treatment. What information should the assistant make certain is on the registration form? What should happen before the treatment plan is implemented?

I would say making sure all the information on the patient's form is correct & that you have number of relative or someone close to them to call in case of emergency. also probably whether they'll be able to afford the treatment or how will they pay for it /what insurance? stuff like that :)
