We add together the cost for the clubs, bag, balls, tees, shirt and shoes.  We then multiply it by 1.065 (6.5%=6.5/100=0.065; however we add 100% of the cost to that, so 1.065).  We then add 10*12 to this, since he is paying $10 for 12 weeks.  This comes to $466.13.
